Client understands mold and microbial growth are moisture-driven conditions. If the underlying moisture source is not corrected, recurrence may occur and is not the responsibility of StressFree Mold Removal.

Client authorizes StressFree Mold Removal to communicate directly with insurance carriers, adjusters, agents, and related parties. Client remains fully responsible for all charges, deductibles, non-covered services, unpaid balances, insurance delays, and denials regardless of insurance outcome.

Payment Authorization & Service Appointment Agreement

By signing below, Client authorizes StressFree Mold Removal to charge the credit card provided for the $195 Service Appointment Reservation Fee required to confirm and reserve the scheduled service appointment.

Client understands and agrees that:

• The Service Appointment Reservation Fee reserves technician scheduling, travel allocation, inspection time, environmental assessment resources, and administrative processing.
• The full $195 fee will be credited toward the final invoice if Client proceeds with services provided by StressFree Mold Removal.
• If additional authorized services are performed, Client authorizes StressFree Mold Removal to charge any unpaid balance remaining after invoice due dates in accordance with the signed service agreement and payment terms.
• Client represents they are an authorized user of the payment method provided.
• Client authorizes StressFree Mold Removal to securely maintain the payment method on file for billing related to approved services rendered.

By signing below, Client acknowledges they have read, understood, and agreed to the terms above.
