Master Service Authorization

I authorize StressFree Mold Remediation LLC to rely on future written, electronic, email, text, phone, purchase order, or work order requests from authorized representatives of this commercial account for a period of twelve (12) months unless revoked in writing.

Service Terms Acknowledgment

I understand that services do not include reconstruction, structural repairs, engineering, plumbing, roofing, HVAC repair, or correction of moisture sources unless specifically contracted in writing.

Electronic Signature Consent

I agree that my electronic signature is the legal equivalent of my handwritten signature and that I am authorized to execute this Commercial Work Authorization on behalf of the company or property owner listed above.

Authorization to Perform Services

The undersigned Authorized Representative hereby engages StressFree Mold Remediation LLC to perform environmental treatment, mold remediation, sanitation, microbial control, odor mitigation, equipment placement, drying support, disinfecting, and related services as requested for the property identified above.

Client authorizes StressFree Mold Remediation LLC to access the property, inspect affected or related areas, evaluate visible site conditions, document work performed, place equipment where appropriate, apply professional treatment products, and perform commercially reasonable services necessary to complete the requested project.

Client acknowledges that services may include, but are not limited to, air movement, air filtration, hydroxyl treatment, HEPA air scrubbing, antimicrobial application, broad-spectrum disinfectant application, enzymatic treatment, odor control, surface treatment, containment recommendations, documentation, and related project support when applicable.

Approved Commercial Account Authorization

Client acknowledges that StressFree Mold Remediation LLC may rely upon verbal, written, text message, email, purchase order, work order, or electronic authorization requests from Client’s authorized representatives.

For established commercial accounts, this authorization may apply to individual projects, repeat service requests, multiple properties, recurring service needs, and future work requested by Client during the active authorization period.

Unless otherwise revoked in writing, Client authorizes StressFree Mold Remediation LLC to proceed with requested work based on communication from an authorized representative of Client.

Scope Flexibility & Site Conditions

Mold, moisture, contamination, odor, and environmental conditions may change or become more apparent once work begins. Client understands that the exact scope, equipment needs, treatment methods, and project recommendations may be adjusted based on site conditions discovered during service.

Services may be modified based on access, safety, moisture, odor, visible contamination, or environmental conditions.
Additional equipment or treatment may be recommended when conditions require it.
Areas not visible, accessible, disclosed, or included in the original request may require separate authorization or additional billing.
StressFree Mold Remediation LLC does not guarantee correction of hidden moisture sources unless specifically contracted in writing.

Change Orders & Additional Work

Any services requested beyond the original scope may be invoiced separately and shall become due under the same commercial payment terms unless otherwise agreed in writing.

Additional work may include expanded treatment areas, additional equipment days, repeat applications, emergency response, after-hours service, disposal, access coordination, documentation, certificate preparation, return visits, or services requested by Client after the original project begins.

Payment Terms

Approved Commercial Accounts

Payment terms may be extended to approved commercial clients. Unless otherwise stated in writing, invoices are due according to the terms shown on the invoice or written agreement.

Recommended Terms

Standard approved account terms: Net 15, unless otherwise approved by StressFree Mold Remediation LLC.

Client agrees to pay all approved charges for labor, materials, equipment, treatment, documentation, mobilization, project management, and related services provided by StressFree Mold Remediation LLC.

Master Service Agreement Option

For repeat commercial clients, this authorization may function as a Master Service Authorization for a twelve (12) month period from the date of signature unless replaced, updated, or revoked in writing.

During that period, Client may authorize additional projects by text, email, phone, purchase order, work order, or other written communication from an authorized representative.

Each individual property, project, or work order may be invoiced separately unless otherwise agreed.

Important Service Limitations

Services do not include reconstruction, structural repair, engineering, plumbing, roofing, HVAC repair, or source correction unless specifically contracted in writing.
StressFree Mold Remediation LLC does not provide medical, legal, insurance, engineering, or real estate advice.
Any warranty or guarantee applies only to the written terms provided for the specific project.
Client understands that untreated moisture sources, leaks, humidity, poor ventilation, or new water intrusion may cause future conditions to return.
Client is responsible for confirming ownership, access authority, tenant coordination, utility access, and site permissions.

Limited Liability

To the fullest extent permitted by law, StressFree Mold Remediation LLC’s liability for any claim, dispute, damage, or alleged loss arising from a specific project shall not exceed the amount paid by Client for the specific project giving rise to the claim.

StressFree Mold Remediation LLC shall not be responsible for pre-existing conditions, hidden defects, concealed contamination, unrelated moisture sources, structural issues, occupant health claims, business interruption, loss of use, lost profits, or conditions outside the agreed scope of work.

Documentation & Certificate of Completion

When applicable, StressFree Mold Remediation LLC may provide project documentation, service notes, photos, treatment summaries, invoice records, and/or a Certificate of Completion.

Certificates and documentation are intended to confirm work performed by StressFree Mold Remediation LLC. They are not a guarantee that unrelated areas, hidden areas, future moisture events, or work performed by others are free from mold, moisture, odor, or environmental concerns.
